Primary Care Networks (PCNs) are a key part of the NHS long-term plan, bringing GP practices together to form networks covering 30,000 to 50,000 patients. 

To meet the needs, GP practices are working together with the community mental health, social care, pharmacy, hospitals and voluntary services to provide care, health and wellbeing of the local population. 

As part of a PCN, GP Practices are able to recruit multi-disciplinary teams including pharmacists, physiotherapists, paramedics, physician associates and social prescribing support workers freeing up time family doctors to focus on more complexed health needs. 

Sevenfields PCN provides NHS services and works collaboratively with seven neighbouring practices:

  • Burnt Ash Surgery
  • Downham Family Medical Practice
  • ICO Health Group
  • Novum Healthcare
  • Oakview Family Practice
  • Torridon Road Surgery
  • Parkview Surgery.

The PCN currently provides extended hours services giving access to GP and Nurse appointments during:

  1. 6.30pm to 8.00pm on weekdays
  2. 9.00am to 5.00pm on Saturdays
